This agreement sets forth the terms, conditions, and stipulations whereby the following parties have committed to a booking engagement:

Performance & Billing:
The Disc Jockey agrees to play recorded materials between the hours of ________________and_________________. If the Disc Jockey is requested and/or forced to extend the time limit of his performance, a higher fee WILL be added, which is ________per hour, payable before the extended time begins.
Payment Terms:
Client shall pay the Disc Jockey the amount outlined as "Disc Jockey's Fee" in exchange for said number of hours of service.
-A deposit in the amount outlined as "Deposit" must be paid to the Disc Jockey sixty (60) days prior to the event. *(The deposit is only refunded if disc jockey cancels, explained below.)
-The remaining balance must be paid, in cash or check, immediately upon Disc Jockey*s arrival at the event and before the Disc Jockey performs.
Deposits and Confirmation:
- Disc Jockey is not confirmed for the event until this signed agreement and the outlined deposit has been received.
- Client must return this signed agreement via email or mail to the Disc Jockey no later than sixty (60) days prior to the event.
- Disc Jockey must receive the deposit no later than 60 (sixty) days prior to the event.
- Disc Jockeys name cannot be used in any advertisements or promotions of the event until this signed agreement and the deposit has been received.
Cancellations:
A cancellation will not be accepted within fifteen (15) days prior to the date of the event. If the event is cancelled within this window of time the client is responsible to pay the disc Jockey the balance due. Once the Disc Jockey arrives at the venue, and is paid the remaining balance due, then by the terms agreed upon in this contract, the Disc jockey will perform services at the event. Disc Jockey will not be held responsible for the inability to perform for the following reasons:
- Police or Government Official closing down location of event
- Location/Promoter losing license
- Facility improperly equipped (ie: no electricity)
- Fire, War, or Act of God
*If the Disc Jockey must cancel for any other reason than those mentioned above, then the Disc Jockey shall refund 100% of the deposit.
Travel & Accommodations:
If the event is located outside of Western Pennsylvania, the Client shall provide and pay for the Disc Jockey's gasoline costs. Client shall also provide Disc Jockey with a pre-paid hotel, for the duration of the Disc Jockey's stay. Confirmation of reservation and prepayment must be faxed or emailed to the Disc Jockey.
Equipment Requirements:
The event site shall provide the Disc Jockey with the following equipment in good working order:
- Two 120 volt receptacles located on two separate circuits.
- A banquet table and two chairs
